Below are the forms that are required when setting up a new Distributor.

1.  Credit Application

      • Required for all new Distributors.  Click on “Download Resources” button below
      • Or Click CREDIT-APPLICATION LINK to view and download a copy of this form

2.  Sales and Use Tax Exempt Form

      • Required for all new Distributors.  This form is supplied by the Distributor.
      • Return with the filled out Credit Application to avoid delays

3.  Personal Guarantee Form

      • If the business is an LLC or less than 2 years in business, this form must also be filled out by the Distributor
